Medicare Annual Wellness Visit or a regular doctor visit?

Understand the difference between Medicare wellness visits, routine physicals and problem visits before scheduling care in Fairfax, VA.

Published October 8, 2026 · General health information

Appointment names can be confusing, especially when Medicare uses a term that sounds like a regular checkup. An Annual Wellness Visit, a routine physical examination and a visit for a medical concern have different purposes. Knowing which conversation you want to have helps the office schedule the right appointment and gives you a chance to check coverage before you arrive.

A wellness visit focuses on prevention planning

Medicare's Annual Wellness Visit is designed to develop or update a personalized prevention plan. It is not the same as a routine physical examination. The conversation may cover your health history, medicines, risk factors and preventive-care needs. You may be asked to complete a health risk assessment. The goal of the appointment is to organize the prevention discussion and next steps, rather than to promise a complete evaluation of every concern in one visit. Ask the office whether this is the service you are scheduling.

A problem visit addresses a specific concern

A regular office appointment may focus on a symptom, follow-up after care elsewhere, medicine questions or an ongoing condition. When you call, explain the main reason you want to see the doctor. You do not need to diagnose the problem yourself; a plain description helps the team choose the right appointment. Tell the office if more than one issue needs attention. A visit for a medical problem is not automatically covered in the same way as a preventive wellness service, even if both happen at the same office.

A routine physical is another distinct service

A physical examination involves an assessment suited to your history and needs, and it may include discussion of testing or screening. The word annual does not make every physical a Medicare Annual Wellness Visit. Do not assume an appointment is free simply because it is described as a checkup. If you want an examination as well as a prevention-plan conversation, say so before the visit. Ask which services the doctor recommends, how they are billed and whether they fall under your particular insurance benefits.

Confirm eligibility and costs before booking

Medicare Part B covers eligible yearly wellness visits once every 12 months. The first visit has timing requirements related to Part B enrollment and a previous Welcome to Medicare visit. When the provider accepts assignment, the wellness benefit can have no patient charge, but additional services may have costs. Routine physical examinations are not covered under this wellness benefit. Your coverage may also depend on whether you have Original Medicare, Medicare Advantage or other insurance. Check the details with the office and your plan rather than relying on the appointment name alone.

Prepare questions for the prevention discussion

Bring a list of your medicines, clinicians and recent screening dates, along with any paperwork the office requests. Write down the preventive-care questions you want to discuss. You might ask which screenings are due, how to organize follow-up or what information another specialist needs. If you use a caregiver or family member to help with appointments, you can ask how to involve them with your permission. Keep your questions specific enough to be answered, and ask for a written outline of the next steps when that would be useful.

Mention symptoms when you call

A wellness visit should not become a reason to delay care for a new concern. Tell the office about the main problem you want addressed so the team can advise on scheduling. If an additional medical service is provided during a wellness appointment, it may be billed separately. Asking about this beforehand gives you and the office a shared understanding of the visit. For severe or sudden emergency symptoms, call 911. An office appointment, whether called wellness or routine, is not a substitute for emergency care.
